Is Milk for Kidney Stones Safe?

To understand how to balance your dairy intake safely, we spoke with Dr Arpit Srivastava, Consultant Nephrologist, Regency Hospital, Gorakhpur. According to Dr Srivastava, moderate milk consumption is generally safe and can even be beneficial for people prone to kidney stones. He explains, “Milk is a good source of dietary calcium, which plays an important role in maintaining bone health and balancing minerals in the body. Adequate calcium intake from food sources can help bind oxalate in the intestine and reduce its absorption, thereby lowering the risk of calcium oxalate stone formation.”

Calcium oxalate stones are the most common type of kidney stones, and maintaining balanced calcium intake may help lower the risk.

A study published in the journal Frontiers in Nutrition investigated the relationship between milk consumption and the risk of developing kidney stones among over 24,000 US adults. By analysing data from the National Health and Nutrition Examination Survey (NHANES), researchers found that people who drank milk more frequently generally had a lower risk of kidney stones.

According to the study, this protective effect was particularly significant in women, where regular milk intake was linked to a 14% reduction in risk. The researchers believe the calcium in milk helps by binding to oxalates in the digestive tract, preventing them from forming stones in the kidneys.

Does Calcium in Milk Increase or Reduce Stone Risk?

Many people mistakenly believe that consuming calcium-rich foods automatically leads to kidney stones. However, research suggests the opposite when calcium comes from food sources.

Dr Srivastava says, “Dietary calcium from milk usually helps prevent kidney stones rather than cause them. When calcium is consumed through food, it binds with oxalate in the digestive tract and prevents excess oxalate from entering the urine.”

However, he notes that excessive calcium supplements without medical supervision may increase the risk of stones. For this reason, experts often recommend obtaining calcium through natural foods like milk and dairy products.

As per a study in Annals of Internal Medicine, while dietary calcium from milk is protective, calcium supplements taken without medical supervision can significantly increase the risk of kidney stones. This occurs because supplements often cause a ‘spike’ in calcium levels in the urine. If these supplements are taken between meals, there is no food (oxalate) in the digestive tract for the calcium to bind to, leaving the excess calcium to be filtered by the kidneys, where it can crystallise into stones.

It was also found that calcium supplements, especially when taken between meals, increase stone risk by 20%. Without food in the gut, supplemental calcium is rapidly absorbed into the bloodstream and filtered into the urine. This creates a ‘calcium spike’, which promotes crystallisation in the kidneys rather than providing the protective ‘gut-binding’ effect of dairy.

How Much Milk Is Safe for Kidney Stone Patients?

For individuals prone to kidney stones, moderation is key. Dr Srivastava explains that two to three servings of dairy products per day, such as milk, curd, or yoghurt, are generally considered safe.

Maintaining a balanced diet with adequate water intake is equally important because hydration helps dilute minerals in urine and lowers the chances of stone formation.

The impact of milk on kidney stones depends heavily on the chemical composition of the stone. While milk is often feared due to its calcium content, modern research shows it is protective for the most common types, but requires caution for others.

1. Calcium Oxalate Stones (Most Common)

Milk is highly beneficial for this type. Most kidney stones are formed when oxalate binds to calcium in the kidneys. When you drink milk, the calcium binds to oxalates in your stomach and intestines instead. As per a study published in Frontiers in Nutrition, this binding happens before reaching the kidneys. The crystals are passed safely through the stool rather than forming stones in the urinary tract.

2. Calcium Phosphate Stones

The impact of milk on calcium phosphate stones is complex due to urine alkalinity. While milk is a primary source of dietary calcium, these stones typically form in high-pH (alkaline) environments.

The National Kidney Foundation states that low-fat dairy is an ideal protein substitute to reduce stone risk by lowering the body's overall acid load and diluting urinary waste. High-purine diets raise uric acid levels, but dairy actually helps the kidneys excrete uric acid more efficiently. 

4. Struvite Stones

Struvite stones, often called "infection stones," are primarily caused by urea-splitting bacteria like Proteus rather than dietary factors. According to a systematic review by the Turkish Association of Urology, their formation is driven by high urine alkalinity and ammonia levels resulting from chronic urinary tract infections. Consequently, milk consumption has no direct impact on causing or preventing these stones, as management focuses on antibiotic therapy and surgical removal.

Is Low-Fat Milk a Better Choice?

Low-fat or skim milk can be a good option, especially for people who are also managing weight or heart health. Dr Srivastava notes, “The calcium content in low-fat and skim milk is similar to that in full-fat milk, so they still provide the benefits of dietary calcium.”

This means that avoiding calcium-rich foods like milk may lead to higher oxalate levels in urine, increasing the chances of calcium oxalate stones.

Scientific research supports the importance of balanced calcium intake. According to a study on Urology, adequate dietary calcium may help reduce the risk of calcium oxalate kidney stones by binding oxalate in the gut before it reaches the kidneys. The research also highlights that combining proper calcium intake with good hydration and a balanced diet can help lower the risk of stone recurrence.

A study in the Turkish Journal of Urology found that simply increasing overall fluid intake significantly reduces stone risk by diluting the urine. However, the type of drink matters: water low in calcium and certain juices, such as orange, grapefruit, and apple, help lower the risk of stone crystallisation. 

According to the study, conversely, drinks such as cranberry juice might increase the risk of certain types of stones. Staying well hydrated with appropriate fluids is a simple yet powerful way to prevent kidney stone disease.

Safety Concerns for Chronic Kidney Disease (CKD) Patients

While milk is beneficial for most people with kidney stones, patients with Chronic Kidney Disease (CKD) must be much more cautious. A study published in the Annual Review of Nutrition explains that as kidney function declines, the body struggles to filter out phosphorus and potassium, both of which are high in milk. 

This study highlights that excessive phosphorus intake in CKD patients can lead to "positive phosphorus balance," which triggers hormonal imbalances that pull calcium out of the bones, making them brittle and causing dangerous calcium deposits in the blood vessels and heart. Therefore, patients with advanced CKD are often advised to strictly limit milk and dairy to prevent these life-threatening complications.
