The honest answer, according to dermatologists, sits somewhere in between.”
She continues, “A pillowcase does not cause acne on its own in the way that hormones, genetics or clogged pores do. However, it can absolutely contribute to breakouts, particularly in people who are already acne-prone. Over the course of a night, a pillowcase accumulates a mixture of sweat, saliva, natural facial oils, dead skin cells, dust and residue from hair products. Left unwashed, this build-up transfers back onto the skin repeatedly, night after night, creating an environment where clogged pores and inflammation are more likely to develop.”

What Study Says About Impact of Pillowcases on Acne
A research published by the National Library of Medicine, conducted a clinical trial to understand if the type of pillowcase you sleep on impacts facial acne. The research involved participants aged twelve and older, who already suffered from mild to moderate acne breakouts.
For twelve weeks, half of the participants were asked to sleep on a standard cotton pillowcase every night. The other half used a specially designed silk-like pillowcase made from synthetic fibers with built-in antimicrobial properties. When the face presses against the same fabric for six to eight hours, that oil, along with any product residue from moisturisers or serums, sits directly against the pores. Add in hair oils, dry shampoo and styling products that transfer from the hair to the pillow, and the surface becomes a breeding ground for bacteria.
Myth 2: Another myth is that switching to silk pillowcases alone will completely solve acne concerns. While silk does reduce friction and absorbs less product than cotton, it is not a substitute for regular washing or a solution for acne caused by hormonal or internal factors.

- A breathable, easy-to-wash cotton or silk pillowcase is preferable to synthetic fabrics that trap heat and oil.
- Cleansing the face thoroughly before bed, rather than sleeping in makeup or sunscreen, also limits the amount of residue that reaches the fabric in the first place.
“Ultimately, a pillowcase will not single-handedly cause acne. But for those already prone to breakouts, it can be a quiet, easily fixed contributing factor. A small change in laundry habits, paired with a consistent skincare routine, often makes a noticeable difference over time,” concludes Dr Passi.
