Does Age Affect Twin Pregnancy Chances? We Asked an Expert

While many people think of twins as a rare "double blessing" decided purely by luck, science suggests that your biological clock might actually be stacking the deck. As more women choose to start families later in life, the rate of twin births has risen significantly. But why does a woman in her 30s have a higher chance of conceiving multiples than someone in her early 20s?

To separate the myths from the medical facts, we reached out to Dr Chetna Jain, Director, Dept of Obstetrics & Gynaecology, Cloudnine Group of Hospitals, Sector 14, Gurgaon. She explains that age is one of the most powerful natural factors in the "twinning" phenomenon.

Does a Woman’s Age Really Affect Her Chances of Having Twins?

“Yes, it actually does, especially when it comes to fraternal twins,” explains Dr Chetna Jain. She clarifies that fraternal twins are formed when two different eggs are released and fertilised in the same cycle. “As the woman gets older, their body sometimes releases more than one egg in a single cycle. If both eggs get fertilised, then the woman will conceive twins. These are non-identical twins.”

Fraternal vs. Identical Twins

Dr Jain also adds an important distinction. “Identical twins are mostly random. They are independent of age and hormonal factors.”

• Fraternal Twins (Dizygotic): These come from two different eggs fertilised by two different sperm. Dr Jain explains that as women get older, their bodies are more likely to release more than one egg in a single cycle.
• Identical Twins (Monozygotic): These occur when a single fertilised egg splits into two. Dr Jain clarifies that these are "mostly random and independent" of maternal age.

Why Are Twin Pregnancies More Common After 30 or 35?

According to Dr Jain, the reason lies in how the ovaries respond to hormones as women age. “As women get older, the ovaries do not respond to hormones as strongly as they used to. So the body releases higher levels of a hormone from the brain called FSH, or follicle-stimulating hormone, to help make more eggs.”

She explains that this hormonal push can sometimes cause the ovaries to release more than one egg. “If both eggs get fertilised, it results in fraternal twins.”

Do Hormonal Changes With Age Cause Double Ovulation?

“Yes, that’s the main reason,” says Dr Jain. “These natural hormonal changes, especially higher levels of FSH, can lead to double ovulation. It is completely natural and becomes more common in women in their early to mid-30s.”

She notes that women between 30 and 37 years of age are more likely to conceive twins naturally. “After that, fertility starts to decline. So while the chance of twins rises for a while, getting pregnant itself becomes more difficult later on.”

Role of Fertility Treatments in Twin Pregnancies

Dr Jain highlights another important factor. “Women are more likely to take fertility treatment after the age of 35 because of declining fertility and the urgency to conceive.” She adds, “Fertility treatments or assisted reproductive techniques also increase the chances of twinning, which is why twin pregnancies are more commonly seen in older women.”

What Precautions Should Women Over 35 Expecting Twins Take?

Dr Jain stresses that twin pregnancy is considered high-risk, especially after 35. “That doesn’t mean something will definitely go wrong, but extra care is required.”

She explains the key precautions:

• Early and regular prenatal check-ups are essential
• More ultrasounds are needed, depending on the type of twins
• Identical twins may require scans every two weeks after 20 weeks
• Non-identical twins usually need monthly scans
• Monitoring may increase if complications arise

Higher nutritional needs, including:

• Extra protein
• Iron
• Calcium
• Folic acid

Higher risk of conditions like:

• High blood pressure
• Gestational diabetes
• Preterm labour and preterm delivery

“These women may also require bed rest, uterine relaxant medications, and close medical supervision,” she adds.

A study published in the journal Human Reproduction found that increasing maternal age is strongly associated with a higher rate of fraternal twinning, largely due to elevated FSH levels and changes in ovarian response. The study explains that women in their early to mid-30s show the highest natural twin conception rates before fertility declines.
