In urban households, passive smoking or second-hand smoke exposure has become a growing concern, yet its severe consequences are often narrowly viewed. While most think the only impact of passive smoke is on the lungs and heart, few know that it affects women's menstrual and reproductive health in ways. The toxic chemicals found in tobacco, such as nicotine and carbon monoxide, exert a direct influence on the hormonal cycle, particularly in women.

To understand how and what impacts passive smoking has on women’s menstrual cycles, leading potentially to irregular periods, the editorial team of Onlymyhealth spoke to Dr Aparna Dadwal, Senior Consultant (IVF), Yatharth Super, Speciality Hospital, Greater Noida. Here is what she shared:

What Is Passive Smoking?

In simple terms, passive smoking, also called second-hand smoking, refers to a situation where an individual breathes in other people’s tobacco smoke. When you accidentally inhale tobacco from any source, such as cigarettes, cigars or hookah, it is not only a discomforting experience for nonsmokers, but also a significant detriment to your health.

The chances of being exposed to second-hand smoke increase in social settings like nightclubs, parties, or public spaces. Tobacco smoke emanates from both the smoker’s mouth and the lit end of the product, and according to Cancer Research UK, most tobacco smoke is invisible, spreads easily and can linger in the air for hours. Air pollution has also significantly contributed to passive smoking.

Does Passive Smoking Affect the Menstrual Cycle?

Passive smoking is one of the most serious global public health concerns, exposing individuals to severe diseases in many ways, especially on women’s menstrual health. Dr Aparna Dadwal shared that tobacco smoke contains more than 7000 chemicals, such as nicotine, carbon monoxide and heavy metals, all directly impacting one’s menstrual cycle.

Dr Aparna Dadwal explained, “Passive smoking is the involuntary inhalation of tobacco smoke that is present in the environment due to active smokers. This can lead to irregular menses, worsen dysmenorrhea (the medical term for menstrual pain in the lower abdomen), and cause restlessness.

Dr Dadwal highlighted that exposure to environmental tobacco smoke during adolescence and early days of life can lead to menstrual irregularity. “It lowers the key reproductive hormones, estrogen and progesterone, leading to hormonal disturbances that affect the regularity of the period, and cause dysmenorrhea, weakness, potentially escalating into broader reproductive issues

Just like active smoking, passive smoking can damage both the quality and quantity of eggs, and may even lead to early menopause. Dr Dadwal warns that chemicals in tobacco not only cause an imbalance in hormone production but also shorter cycles and more frequent ovulation.

Does Passive Smoking Equal Direct Smoking?

As far as general health risk is concerned, smoking significantly increases cardiovascular risks, lung cancer risks and other chronic health issues, whether active or passive.

However, Dr Dadwal underlined a crucial distinction, saying, “ In some ways, active smoking is more detrimental because of direct inhalation and long-term contact with toxic chemicals. Over time, this can increase the likelihood of severe chronic diseases and disability.”

Nonetheless, passive smoking still poses severe threats. Reports indicate that it may increase the risk of other cancer types, such as breast cancer, and serious lung conditions like chronic obstructive pulmonary disease (COPD).

Can You Avoid Passive Smoking

Avoiding passive smoking has become quite impossible these days. Whether you are in public space or in a close space with your friends, you will be exposed to passive smoking in any case. Poor air quality in metro cities is also to blame for the rise of tobacco smoke.

Dr Dadwal suggested that one can use a good quality mask, and make a dedicated smoking area with proper ventilation at home to avoid inhaling the toxins.
