Is Hormonal Imbalance The Reason Behind Premature Ejaculation?

Struggling with premature ejaculation? An expert explains if hormonal imbalance is the reason behind it and shares effective ways to treat the root cause.

Premature ejaculation (PE) is one of the most common sexual concerns among men. It occurs when ejaculation happens sooner than desired during sexual activity, often causing frustration, stress, and relationship difficulties. While psychological factors such as anxiety and performance pressure are commonly associated with PE, many men wonder whether hormonal imbalances could also be responsible.

To understand this better, we reached out to Dr Chetna Jain, Director Department of Obstetrics and Gynaecology, Cloudnine Group of Hospitals, Sector 14, Gurgaon.

Is Premature Ejaculation Caused By Hormonal Imbalance?

Dr Jain says that the answer is – Yes. The expert explains, “Hormonal imbalances can contribute to premature ejaculation in some cases, although they are not the sole cause.”

Let us now understand what PE is. “Premature ejaculation is generally defined as ejaculation that occurs within about one minute of penetration or earlier than a person or couple would like. It can be lifelong (present since the first sexual experiences) or acquired later in life,” says Dr Jain. The expert lists the common causes as follows:

  • Performance anxiety
  • Stress and depression
  • Relationship issues
  • Erectile dysfunction
  • Certain medical conditions
  • Hormonal imbalances
  • Nervous system disorders

How Can Hormonal Imbalances Affect Ejaculation?

Dr Jain says that hormones play a crucial role in regulating sexual function, libido, mood, and reproductive health. “When hormone levels become abnormal, sexual performance may be affected,” she adds. On this note, the expert lists the factors related to hormonal imbalance that can impact ejaculation:

1. Testosterone Imbalance

Testosterone is the primary male sex hormone. Low testosterone levels can affect sexual desire, erectile function, and ejaculation patterns. While low testosterone is more commonly linked to reduced libido, some studies suggest that hormonal disturbances involving testosterone may contribute to ejaculation problems in certain men.

2. Thyroid Disorders

Research has shown a strong connection between hyperthyroidism (overactive thyroid) and premature ejaculation. Excess thyroid hormone can alter nerve signaling and sexual function, increasing the likelihood of rapid ejaculation. Treating the thyroid disorder often improves symptoms.

3. Prolactin and Other Hormones

Abnormal levels of prolactin and other reproductive hormones may also influence sexual function and ejaculation control, although this is less common.

Other Factors Often Play a Bigger Role

Dr Jain says that while hormones can contribute to it, many cases of premature ejaculation are related to:

  • Anxiety about sexual performance
  • Lack of sexual experience
  • Relationship stress
  • Guilt or emotional concerns
  • Increased sensitivity of the penis
  • Chronic stress and poor sleep

“Because multiple factors can be involved, a thorough medical evaluation is important when symptoms persist,” she adds.

Tips to Deal with Premature Ejaculation

Dr Jain shares certain tips that can help deal with premature ejaculation:

1. Manage Stress and Anxiety

Stress is one of the biggest contributors to sexual difficulties. Relaxation techniques such as meditation, deep breathing, and mindfulness can help improve control.

2. Practice Pelvic Floor Exercises

Strengthening pelvic floor muscles through kegel exercises may help improve ejaculation control in some men.

3. Try the Start-Stop Technique

This involves stopping sexual stimulation when ejaculation feels imminent and resuming once the urge decreases. Over time, it may help build better control.

4. Maintain a Healthy Lifestyle

Regular exercise, adequate sleep, and a balanced diet support hormone health and overall sexual well-being.

5. Avoid Excess Alcohol and Smoking

These habits can negatively affect sexual performance and reproductive health.

6. Seek Medical Evaluation

If premature ejaculation develops suddenly or is accompanied by symptoms such as reduced libido, fatigue, weight changes, or erectile dysfunction, hormonal testing may be recommended. Conditions such as thyroid disorders or testosterone abnormalities can often be treated effectively.

When Should You See a Doctor?

Dr Jain says that consult a healthcare professional if:

  • Premature ejaculation occurs frequently.
  • It causes distress or relationship problems.
  • Symptoms appear suddenly after years of normal sexual function.
  • There are signs of hormonal imbalance, such as fatigue, mood changes, weight fluctuations, or reduced sex drive.

Conclusion

Hormonal imbalances, particularly thyroid disorders and certain reproductive hormone abnormalities, can contribute to premature ejaculation. However, psychological factors, stress, and lifestyle issues are often more common causes. The good news is that premature ejaculation is a treatable condition. Identifying the underlying cause and adopting healthy lifestyle habits, stress-management techniques, and appropriate medical treatment can significantly improve sexual confidence and satisfaction.
