We often mistake a lingering cough as a stubborn cold or a seasonal allergy. However, medical experts warn that such subtle, recurring respiratory issues are often the early hidden symptoms of asthma. If ignored for long, this may lead to severe health complications. To understand this better, we reached out to our experts, Dr Sunil Kumar K, Lead Consultant,  Interventional Pulmonology, Aster CMI Hospital, Bangalore and Dr Ashok Kumar Singh, Director, Department of Pulmonary and Critical Care Medicine, Regency Health Kanpur.

Dr Sunil says that asthma often develops gradually, and its early symptoms can easily be mistaken for other common health problems, leading to delays in diagnosis and treatment. 

The expert explains, “From a healthcare perspective, one of the earliest signs is a persistent cough, especially at night, early in the morning, or after exercise. Many people assume it is due to a common cold, seasonal allergies, acid reflux, or a lingering respiratory infection.”

Shedding more light on this, Dr Sunil shares some of the other early symptoms of asthma which may mimic other diseases:

  • Wheezing
  • Chest Tightness
  • Shortness of breath
  • Feeling unusually tired during physical activity
  • Frequent coughing while laughing, climbing stairs, or when exposed to dust, smoke, cold air, or strong fragrances. 

“These symptoms may come and go, making them easy to ignore,” adds the expert.

Warning Signs of Asthma To Look Out For

Dr Sunil says that unlike a common cold, asthma symptoms often return repeatedly and may worsen after exposure to specific triggers. 

“Ignoring these warning signs can allow airway inflammation to progress, increasing the risk of severe asthma attacks over time. Anyone experiencing recurring cough, wheezing, or breathing difficulty should seek medical evaluation rather than relying on self-medication,” explains the expert.

According to the Asthma and Allergy Foundation of America (AAFA), here are some of the common signs and symptoms of asthma:

  • Shortness of breath.
  • Cough.
  • Chest tightness or pain.
  • Wheeze.
  • Waking up at night due to asthma symptoms.
  • A drop in your peak flow meter reading.

Why Early Asthma Symptoms Are Often Ignored 

Dr Ashok says that India accounts for 13% of the global asthma burden. “The disparity is so stark that it becomes difficult to comprehend until you learn about its hidden elements. The actual situation shows that these fatalities occur due to underdiagnosis, poor disease control, and limited access to essential medications,” explains the expert.

Speaking about why early asthma signs are often ignored, he says, “Such incidents are happening across millions of households, and the earliest signs of asthma are being explained away as dust allergy, a recurring cold, or the body simply ‘adjusting’ to the air. By the time patients arrive at a pulmonologist's clinic, the disease has often been active and unmanaged for years. This gap does not close by itself, and it does not begin at the hospital. It begins at home, in the decision to wait a little longer before seeking help.”

When a Cough is Never Just a Cough

Dr Ashok says that asthma typically begins with subtle changes in breathing that are easy to overlook or misinterpret in the early stages. Explaining this further, he says, “A dry cough that lasts more than a seasonal cold, together with chest tightness that occurs after climbing stairs and breathlessness that people link to their fitness level and age constitute medical warning signs that people tend to ignore for extended periods.”

He adds, “One of the defining challenges with asthma is that its symptoms are often mistaken for common respiratory problems, leading to underdiagnosis. In India, this is particularly concerning, as many people seek treatment only when symptoms become severe. As a result, what begins as an easily manageable condition can gradually progress into a chronic and more difficult-to-treat disease.”

Factors Leading to Rising Cases of Asthma in India

Dr Ashok says that the problem in India has become bigger due to increasing pollution levels which have changed everything. “Northern regions of the country are experiencing a growing asthma epidemic which results from two factors: rising pollution levels and increasing smoking rates among adolescents. Across urban areas, health problems are caused by deteriorating air quality, which especially affects children and elderly people who develop symptoms that include persistent nighttime coughing, wheezing, difficulty in breathing and chest tightness.”

He continues, “The situation becomes more complex because people face both outdoor and indoor exposure to contaminants. The practice of burning incense inside homes and temples causes airway dysfunction which leads to respiratory problems in people, while exposure to tobacco and biomass smoke increases asthma risk for all adults, especially women. People encounter these exposures throughout their daily activities, yet they do not realise how their standard practices cause the subsequent chest tightness.”

Why Early Diagnosis of Asthma is Important 

Dr Sunil says that early diagnosis through a medical history, physical examination, and lung function tests allows appropriate treatment to begin promptly. The expert adds, “With the right medicines, trigger avoidance, and regular follow-up, most people with asthma can effectively control their symptoms and lead active, healthy lives.”

Adding to this, Dr Ashok says, “Catching asthma early does not demand expensive intervention or specialist access. What it demands is a shift in how patients, families, and primary care doctors understand what a persistent cough or recurring breathlessness is telling them, because the disease is already present, already progressing, and entirely manageable if met at the right time.”
