Does Vitamin C Increase Kidney Stone Risk

Moderate intake of vitamin C is safe, but research shows that higher doses of vitamin C can significantly increase the risk of kidney stones in men, but not women.

"Kidney stones are hard deposits formed from minerals and salts that crystallise in the urine. The most common type of kidney stone is calcium oxalate stones," shares Dr Vishal, Consultant Urologist at Fortis Hospital, Nagarbhavi, Bengaluru. According to a study published in PubMed Central, total and supplemental vitamin C intake is associated with an increased risk of kidney stones in men. When the body metabolises vitamin C, a portion of it is converted into oxalate, a compound that can combine with calcium in urine to form calcium oxalate crystals.

Studies, including one published in the Official Journal of the International Society of Nephrology, Kidney International, have shown that very high doses of vitamin C supplements, more than 1,000 mg per day, may increase urinary oxalate levels. These elevated oxalate levels can increase the risk of stone formation, particularly in individuals who already have a history of kidney stones or are predisposed to them.

NOTE: While the RDA is 90mg and 75mg in men and women, respectively, the tolerable upper intake level set by the NIH is actually 2,000mg.

According to Dr Saurabh Joshi, Director of Urology, Robotics and Kidney Transplant, Yatharth Super Speciality Hospital, Faridabad, “It is important to note that moderate intake of vitamin C is generally safe for most people and does not usually lead to kidney stone formation.”

The Role of Dietary Sources vs. Supplements in Kidney Stone Risk

Vitamin C can be obtained naturally from foods or through dietary supplements. The source of vitamin C may influence its effect on kidney stone risk.

Foods rich in vitamin C, such as lemons, oranges, guavas, strawberries, and kiwis, typically contain moderate amounts of vitamin C along with other nutrients, such as fibre, antioxidants, and potassium. These nutrients help the body metabolise vitamin C efficiently, thereby producing only small amounts of oxalate.

On the contrary, high-dose vitamin C supplements deliver large quantities of vitamin C at once, which may be higher than the daily vitamin C requirements. The excess vitamin C that the body cannot utilise gets converted into oxalate and excreted in the urine, potentially contributing to the formation of kidney stones in people in vulnerable groups.

Research published in JAMA Internal Medicine shows 48,850 men, aged 45 -79 years, were observed for 11 years to understand the link between ascorbic acid (an isolated compound that makes up Vitamin C) and the formation of kidney stones. It was found that ascorbic acid use can increase stone formation risk by two-fold.

“For this reason, many healthcare professionals recommend obtaining vitamin C primarily from natural food sources rather than high-dose supplements, especially for individuals who have previously experienced kidney stones,” quotes Dr Joshi.

There are several misconceptions regarding vitamin C and kidney stone formation.

Myth: Any amount of vitamin C can cause kidney stones.
Fact: Dietary intake of vitamin C within the normal range(<1000mg) does not increase the risk of kidney stones in healthy individuals.

Myth: Citrus fruits should be avoided due to their vitamin C content.
Fact: Citrus fruits contain citrate, a natural compound that can help prevent kidney stones by binding to calcium in the urine and reducing crystal formation. The National Kidney Foundation also supports the fact that citrus fruits might be beneficial for people trying to prevent certain types of kidney stones. Additionally, limes/lemons have higher citrate than oranges.

  • Maintaining adequate intake of dietary calcium(1000-1200mg per day)
  • Reducing excessive salt intake
  • Limiting animal protein consumption
  • Avoiding very high-dose vitamin C supplements without medical advice
  • Limiting the consumption of foods that have high oxalate content, such as spinach and beetroot, particularly in individuals prone to calcium oxalate stones

However, the advice for vitamin C intake is not one-size-fits-all. Some people have primary hyperoxaluria. In simple terms, they are super absorbers of vitamin C, and so even 500mg of vitamin C can be catastrophic for their health. Thus, it is important to know your body and act accordingly.
