To get the facts, we spoke to Dr Swathi Prakash, Consultant - Breast Surgery at the Cancer Centre, Surgical Oncology, Rela Hospital, Chennai, who sheds light on what truly causes sagging and the realistic ways to address it.

Breast sagging, medically known as ptosis, occurs when the skin and supporting tissues of the breast stretch and the breast tissue itself descends. Dr Prakash explains, “Sagging happens because the skin of the breast is being pulled down. The gland and fat inside the breast also droop, and the nipple lowers in position.”
Sagging is classified into different grades based on how low the nipple sits compared to the breast crease. This condition is common as women age, after pregnancy, or with weight changes.

Does Ice Massage Help to Uplift Saggy Breasts?![]()
The short answer: No, not in any meaningful or lasting way. There is a popular belief that ice massages or creams can improve breast shape and firmness. Dr Prakash clarifies, “No, ice and creams do not help uplift saggy breasts because they work only on the skin level. Breast sagging involves the gland and fat tissues pulling down the surface; treatments can’t reverse that.”
While massage and cold therapy may improve skin appearance momentarily (by reducing puffiness or increasing circulation), they cannot reverse the descent of internal breast tissue.
A caution from other sources: vigorous or frequent massaging, especially pulling or pushing, can further stretch skin or weaken connective tissue. Some dermatology sources warn that strong breast massage might contribute to sagging over time.

For those seeking to lift saggy breasts, Dr Prakash recommends surgical options. “You can lift the nipple area alone or perform a breast lift procedure where the breast tissue is reshaped and lifted.”
She adds that surgical treatment addresses the real cause of sagging: “As we age, glandular tissue converts to fat and skin elasticity reduces. Breastfeeding and gradual weight loss can also cause sagging. Wearing a supportive bra is crucial to help prevent worsening.”
Common causes of sagging include:
- Age-related skin and tissue changes
- Breastfeeding—especially if not supported with a good bra afterwards
- Gradual weight loss leading to less volume and sagging
- Gravity and lack of supportive undergarments

Again, the expert answer: No. Dr Prakash states plainly, “Massaging breasts does not help with sagging.” Some general health sources agree: while light, gentle massage may help with comfort or circulation, there's no evidence it can reverse sagging.
What Can Help Firm Breasts?
Besides surgery for those with marked sagging, there are ways to support breast appearance and prevent deterioration. Though you can’t fully reverse ptosis non-surgically, certain habits and methods can help slow sagging or subtly improve appearance. Dr Prakash recommends:
- Strengthening chest wall muscles: Push-ups, chest presses, back exercises
- Improving posture: Holding shoulders back, standing straight
- Wearing a supportive, well-fitting bra (especially for active or large breasts)
- Avoiding rapid, extreme weight loss
- Quitting smoking to preserve skin elasticity
- These steps won’t lift the internal breast tissue itself, but they support the overlying skin and appearance.
Dr Prakash underscores, “These are supplementary measures. Once sagging is present, surgical lifting is the only reliable route.”
A study published in the Journal of Plastic, Reconstructive & Aesthetic Surgery highlights that properly fitted bras can significantly reduce breast movement and stress on ligaments, potentially slowing the progression of sagging. The study emphasises the importance of external support in maintaining breast shape throughout life.
