Have you ever heard of semen allergy? No matter how bizarre it may sound, this is a real condition. It is medically known as human seminal plasma hypersensitivity (HSP). This rare condition affects mostly, and almost exclusively as reported by some data, women. If you want to know how it gets triggered, what causes this, or how to treat it, then this article is for you.
To understand this better, we reached out to Dr Rakhi Rai, Director, Department of Fertility at Cloudnine Group of Hospitals, Noida.
What is Seminal Plasma Hypersensitivity?
Let us first understand what human seminal plasma hypersensitivity is. Answering this, Dr Rai says, “A semen allergy, also known as human seminal plasma hypersensitivity, is a rare allergic reaction to proteins found in semen. As a result of this, reactions can occur in any area that comes in contact with semen. However, it’s most common around the genitals. This allergy can be diagnosed with a skin test.”
The expert adds that semen allergies can be of two types:
- Localised semen allergy: Only the body parts that directly come in contact with semen are affected. People with localised semen allergies may experience a burning or stinging sensation in the genital area, hands or lips.
- Systemic semen allergy: This type of semen allergy impacts your entire body. Some of its symptoms include difficulty breathing, swelling of the lips or tongue, hives or experiencing signs of anaphylactic shock.

6 Signs You May Have Seminal Plasma Hypersensitivity
Dr Rai says that some of the most common symptoms of a semen allergy include:
- Itching, redness, swelling and burning on your skin.
- Hives.
- Swelling of the lips and tongue.
- Difficulty breathing, wheezing
- Dizziness.
- Diarrhoea, nausea or vomiting.
- Painful intercourse or pain after intercourse.
- Nausea/vomiting.
So what causes semen allergy? “This may occur due to your immune system’s reaction to certain proteins which are found in semen. Doctors are not entirely sure what causes the reaction but they believe that medications, food sensitivities or hormonal events such as pregnancy and menopause may play a role,” shares the expert.

What Study Says About Semen Allergy
A 2024 research, published in the American Journal of Reproductive Immunology, attempted to understand the condition seminal plasma hypersensitivity better. The researchers analysed 53 scientific articles to summarise its common signs, diagnostic methods, and available treatments.
The review discovered that allergic reactions typically fall into two categories: localised reactions and systemic reactions. It added that to accurately diagnose a semen allergy, doctors depend heavily on a patient's detailed symptom history alongside specific skin prick tests.
The study confirmed that using a condom successfully prevents these allergic flare-ups as it acts as a physical barrier. For couples who are trying to conceive, the research adds that being allergic to semen does not directly cause infertility. Therefore, if a woman wants to get pregnant safely, doctors may suggest desensitisation therapies or specialised artificial insemination techniques to prevent the allergic response.

How To Diagnose and Treat Semen Allergy?
Dr Rai says that symptoms of HSP are often similar to other conditions such as sexually transmitted infections (STIs), vaginitis, or yeast infections. She adds, “This is why it is important to diagnose it correctly.” Dr Rai says that a doctor takes a medical history and assesses symptoms to effectively diagnose HSP; they may carry out tests such as:
- Vaginal examination.
- Vaginal swabs to check for any bacterial or yeast infections.
- Nucleic acid amplification tests (NAATs), which help to rule out any STIs.
- Blood tests.
“A doctor may also perform a skin prick test using a sample of semen proteins from the patient's sexual partner to check for any reaction,” adds the expert.
Dr Rai says that coming to its treatment, the options include:
- Using a barrier method such as a condom.
- Taking an oral antihistamine 30–60 minutes before sex.
- Having an epinephrine autoinjector, such as an EpiPen, to hand in case of a severe allergic reaction.
- Desensitisation treatment. This may include gradual dilutions of semen from a sexual partner into the vagina or injections of semen proteins under the skin.

Conclusion
Seminal plasma hypersensitivity is a rare condition. It can cause significant physical discomfort and distress during or after intimacy. Since its localised symptoms often closely mimic common issues such as yeast infections or STIs, getting the right medical diagnosis is a crucial first step. But the good news is that with effective management strategies, ranging from simple barrier methods to targeted desensitisation therapies, couples can safely navigate this allergy without compromising their health or fertility goals.
