A good night’s sleep is often described as one of the simplest ways to support healthy skin. Yet, there is an interesting contradiction: the way a person sleeps may contribute to the appearance of certain facial lines over time. These are commonly referred to as ‘sleep wrinkles’ or sleep lines. To understand how your sleeping position may actually age your face, and how you can prevent it, we interacted with a top dermatologist.

Let us take a look at what Dr Ruben Bhasin Passi, Senior Consultant, Dermatology, CK Birla Hospital, Gurgaon, has to say about this.  

What Are Sleep Wrinkles? 

Let us begin by understanding what sleep wrinkles are. Answering this, Dr Passi explains that sleep wrinkles are lines created by the way the face is compressed against a pillow during sleep. Shedding more light on this, she says, “When a person sleeps on their side or stomach, the skin may be pushed, folded or stretched between the pillow and the underlying facial structures.”

The expert further explains, “In younger skin, these lines usually disappear relatively quickly after waking. As the skin ages, however, collagen and elastin gradually decline, and the skin becomes less capable of returning to its original position. Consequently, lines caused by repeated mechanical stress can become more persistent. This is different from expression lines, which are produced by repeated muscle movements such as smiling, frowning or squinting.”

Dr Passi continues, “Unlike the wrinkles that develop primarily because of ageing and repeated facial expressions, sleep wrinkles are associated with prolonged mechanical pressure, compression and stretching of the skin while a person sleeps. This does not mean that sleeping on one side will suddenly make half of the face look older. However, repeated pressure on the same area night after night may contribute to the formation of lines, particularly as the skin becomes less elastic with age.”

Can Sleeping On One Side Age Your Face Faster?

Dr Passi says that the answer to this question is more nuanced than social media claims suggest. Adding to this, the expert says, “Sleeping consistently on one side can expose that side of the face to greater mechanical pressure. Over many years, this may contribute to the development of sleep-related lines. However, it would be an exaggeration to say that a particular sleeping position will rapidly or dramatically age one side of the face.”

She continues, “Sun exposure, genetics, smoking, environmental pollution, loss of skin elasticity, hormonal changes and the natural ageing process generally have a much greater influence on long-term skin ageing. Therefore, someone who wakes up with a few temporary lines on one cheek does not need to panic.”

Can Back Sleeping Prevent Sleep Wrinkles?

Dr Passi says that sleeping on the back reduces direct contact between the face and the pillow and therefore minimises mechanical compression. “From a purely dermatological perspective, it can be a useful habit for people concerned about sleep lines. However, not everyone can comfortably sleep on their back. Trying to force a particular sleeping position can also disturb sleep quality, and poor sleep itself can negatively affect overall skin health,” adds the expert.

She further explains, “For those who naturally sleep on their side, there is no need to abandon a comfortable and healthy sleeping pattern solely because of fear of wrinkles.”

Tips To Prevent Sleep Wrinkles

On this note, Dr Passi shares some tips to prevent sleep wrinkles:

  • Good skin care remains far more important than obsessing over sleeping position.
  • Apply a broad-spectrum sunscreen with an appropriate SPF every day, including when the weather appears cloudy. Ultraviolet exposure is one of the major contributors to premature skin ageing.
  • Regular moisturisation can also support the skin barrier and reduce dryness, which can make fine lines appear more noticeable.
  • Ingredients such as retinoids may help improve fine lines over time, but they should be introduced appropriately because they can cause irritation, particularly in sensitive skin.
  • Using a clean, smooth pillowcase and changing it regularly is sensible for general skin hygiene, although there is no miracle pillowcase that can prevent ageing.

Conclusion

Sleep wrinkles are real, but they are often exaggerated online. Sleeping on one side does not suddenly age half the face, nor does a person need to completely change a comfortable sleeping position because of a few morning lines. The bigger picture matters more: protecting the skin from ultraviolet radiation, avoiding smoking, maintaining adequate moisturisation, eating a balanced diet and following a sensible skincare routine have a much stronger role in preserving skin health. A few pillow lines in the morning are therefore not a warning sign of accelerated ageing. In most cases, they are simply evidence that the skin has spent several hours in the same position, and that is hardly a dermatological emergency.
