Whether it’s feeling constantly tired, dealing with weight changes, or noticing shifts in mood, thyroid imbalances can have a profound impact on a woman’s libido and sexual well-being. For those living with hypothyroidism (underactive thyroid) or hyperthyroidism (overactive thyroid), sexual function can be disrupted in various ways, affecting desire, arousal, and even comfort during intercourse. We spoke to our expert Dr Vaishali Sharma MBBS, MD (AIIMS) Obstetrics and Gynaecology, Laparoscopic Surgeon, who explained how these disorders lower sex drive in women.

Thyroid Health and Sexual Function

"The thyroid gland produces hormones that help regulate vital bodily processes, including the metabolism of fats, proteins, and carbohydrates. These hormones also influence the reproductive system, which is why any disruption to thyroid function can have a profound effect on a woman’s sexual health," said Dr Sharma.

While thyroid disorders are common, their impact on libido is often overlooked. In many cases, thyroid imbalances manifest through symptoms that indirectly affect sexual desire, such as fatigue, weight changes, and mood disturbances.

Women with hypothyroidism and hyperthyroidism showed impairments in desire, arousal/lubrication, orgasm, satisfaction, and pain during intercourse. These conditions affect circulating sex hormone levels through peripheral and central mechanisms, potentially causing psychiatric and autonomic disruptions that hinder sexual function, as stated in a 2019 review.

Hypothyroidism and Libido

In women, hypothyroidism is more commonly associated with a decreased sex drive. The thyroid gland produces hormones that regulate metabolism, and when it’s underactive, the body’s systems, including reproductive functions, tend to slow down.

"Women with hypothyroidism may experience a range of symptoms, such as fatigue, weight gain, depression, and reduced interest in sex. The drop in thyroid hormone levels can lead to lower levels of oestrogen, a hormone vital for sexual arousal," added Dr Sharma. The imbalance in these hormones contributes to reduced libido, vaginal dryness, and discomfort during intercourse, further diminishing sexual desire.

Hyperthyroidism and Libido

On the other hand, hyperthyroidism, though less common, can also affect libido, but in a different way. Excess thyroid hormone can cause mood swings, anxiety, and irritability, which indirectly affect sexual desire.

Rapid heart rate, sleep disturbances, and increased metabolism can make a woman feel overwhelmed, reducing her interest in sex. Additionally, hormonal imbalances can affect fertility and menstrual cycles, further complicating sexual health.

Managing Thyroid-Related Sexual Dysfunction

If thyroid imbalances are contributing to a decrease in libido, the good news is that it is possible to manage and treat the underlying thyroid condition. "Restoring proper thyroid function through medication, lifestyle changes, and regular monitoring is often the first step in improving sexual health. For women with hypothyroidism, thyroid hormone replacement therapy can help normalise hormone levels, which may gradually improve libido and sexual function," highlighted Dr Sharma.

In addition to medical treatment, maintaining a healthy lifestyle—regular exercise, a balanced diet, and stress management techniques—can support overall well-being and improve sexual function.

It's essential for women experiencing unexplained changes in their sex drive, energy levels, or mood to get their thyroid function checked. Thyroid-related sexual dysfunction can be treated effectively, and understanding the role of the thyroid in reproductive health is key to maintaining a fulfilling sexual life.
