Dr Sharadhi says that caffeine and sugar do not directly cause bipolar disorder or create a manic episode on their own.
The expert explains, “But they can act as triggers or worsen symptoms in some people who already have bipolar disorder, especially those who are sensitive to stimulants or have poor sleep patterns; caffeine is a stimulant that increases alertness and can raise heart rate, anxiety, and restlessness.”

How Caffeine Impacts Manic Episodes
Dr Sharadhi says, “Consuming large amounts of coffee, energy drinks, or other caffeinated products may contribute to sleep disruption, which is a major risk factor for triggering mood changes, including mania or hypomania in vulnerable individuals.”
A 2020 study, published in the medical journal Bipolar Disorders, examined existing research to understand if drinking caffeine can impact those diagnosed with bipolar disorder. According to the research, consuming caffeine did cause a shift into a manic or hypomanic state for some patients.
The scientists believe that the sudden increase in caffeine can indeed trigger mania by acting as a direct stimulant on the brain. Caffeine is also known to negatively affect normal sleep patterns, which is a major known trigger for bipolar manic episodes. Additionally, the study discovered that caffeine can interfere with how the body processes lithium, a common mood-stabilising medication. Caffeine suppresses lithium levels in the blood. That is when the medication becomes less effective at preventing these manic highs. However, the researchers also added that the current evidence is not entirely conclusive. Moreover, long-term research is needed to prove whether caffeine can directly cause mania. Nevertheless, bipolar patients are generally advised to monitor their caffeine intake carefully.

How Sugar Impacts Manic Episodes
The expert says that sugar itself is not proven to directly trigger mania, but consuming large amounts of sugary foods or drinks can cause rapid changes in blood glucose levels.
Dr Sharadhi further explains, “This can lead to temporary energy boosts followed by tiredness, mood swings, irritability, and changes in concentration. In some people with bipolar disorder, these fluctuations, combined with an unhealthy diet, stress, or lack of sleep, may make it harder to maintain stable moods, while excessive caffeine and sugar intake may also worsen anxiety, agitation, impulsive behaviour, and racing thoughts, which can overlap with symptoms seen during a manic episode.”
A 2025 study, published in The Journal of Nutrition, Health & Aging, examined the dietary habits of nearly 1,70,000 people to understand how different types of sugar impacts common mental health conditions.
The researchers identified bipolar disorder as a major mental illness that is influenced by diet. Despite not directly measuring mania, the findings provide strong biological clues about how heavy sugar consumption can theoretically destabilise mood in bipolar patients. The researchers also discovered that diets high in added sugars, sucrose, and total sugars drastically raise the risk of developing severe mood disorders.
It noted that consuming excessive amounts of sugar leads to insulin resistance, which directly harms normal brain function and worsens psychiatric symptoms. Moreover, the study added that a high-sugar diet triggers intense inflammatory responses throughout the body and brain, a condition known to worsen mood episodes. While this research does not definitively prove that sugar directly triggers a bipolar manic episode, it clearly demonstrates that high-sugar diets create an inflammatory, unbalanced brain environment that makes severe mood instability much more likely.

Tips To Prevent Sugar and Caffeine-induced Manic Episode
Dr Sharadhi says that people with bipolar disorder can manage their risk of mood episodes by:
- Maintaining a regular sleep schedule.
- Taking prescribed medications consistently.
- Limiting excessive caffeine and high-sugar foods.
- Staying physically active.
- Eating balanced meals with enough protein and fibre.
- Tracking personal triggers that affect their mood.
“It is important to remember that triggers are different for each person. Sudden changes such as unusually high energy, reduced need for sleep, rapid talking, increased confidence, impulsive decisions, or risky behaviour should be discussed with a mental health professional, as early recognition and timely support can help prevent a severe manic episode,” concludes the expert.
