Fatty liver disease often develops quietly. Many people have excess fat in the liver without experiencing clear symptoms, particularly in the early stages. This is why the condition can sometimes progress without being noticed.
In some people, however, ongoing liver damage can lead to symptoms or changes in health that should not be ignored. Recognising these warning signs early can help you seek medical advice before serious complications develop.
Here are five signs, according to Dr Neeraj Dhamija, Senior Consultant - Laparoscopic, Laser & General Surgery at Sir Ganga Ram Hospital, New Delhi, that may suggest your fatty liver disease is worsening.
1. Increasing Tiredness And Weakness
Feeling unusually tired can be one of the less specific symptoms associated with liver problems.
According to the American Liver Foundation, extreme tiredness is a prominent warning sign of Metabolic Dysfunction–Associated Steatotic Liver Disease (MASLD), formerly called NAFLD.
As liver function declines, you may experience persistent fatigue or weakness that doesn't improve with normal rest. You might find everyday activities more exhausting than usual or notice that your energy levels have dropped without an obvious explanation.
However, tiredness can have many causes, including poor sleep, anaemia, thyroid problems, stress and nutritional deficiencies. Therefore, fatigue alone does not mean that fatty liver disease has progressed.
NOTE: If tiredness is persistent or accompanied by other symptoms, it is worth discussing it with a doctor.
2. Pain Or Discomfort In The Upper Right Abdomen
The liver is located in the upper right part of the abdomen, beneath the ribs.
According to a study published in the Journal of Clinical Medicine, some people with fatty liver disease may experience discomfort, pressure, or a dull ache in this area. This can happen when the liver becomes enlarged or inflamed.
Persistent or worsening discomfort should not be ignored, particularly if you already have diagnosed fatty liver disease. At the same time, pain in this region can have several other causes, including gallbladder and digestive problems.
NOTE: A medical assessment is needed to determine the actual cause.

3. Yellowing Of The Eyes Or Skin
Yellow skin or yellowing of the whites of the eyes, known as jaundice, is a more concerning sign.
It can occur when the liver is no longer processing bilirubin effectively. Bilirubin is a yellow substance produced when red blood cells are broken down.
“Jaundice is not a typical feature of simple fatty liver and may indicate significant liver dysfunction or another liver-related problem,” shares Dr Dhamija.
NOTE: If you notice yellowing of your eyes or skin, particularly alongside dark urine or pale stools, seek medical attention promptly.
4. Swelling In The Legs, Ankles, or Abdomen
Swelling can develop when advanced liver disease affects the body's ability to regulate fluids and proteins.
You may notice swelling around your ankles and legs. In more advanced cases, fluid can accumulate in the abdomen, causing it to become noticeably swollen. This is known as ascites.
These symptoms can have several causes, including heart, kidney, and other medical conditions. But in someone with known liver disease, new or increasing swelling requires medical assessment.
Sudden or significant abdominal swelling should not be assumed to be simply weight gain.
5. Easy Bruising Or Bleeding
A healthy liver plays an important role in producing proteins involved in blood clotting.
When significant liver damage occurs, the body's ability to produce some of these proteins can become impaired. This may make a person bruise more easily or bleed for longer than usual.
Unexplained or frequent bruising, nosebleeds, or other unusual bleeding should therefore be discussed with a doctor.
These symptoms do not automatically mean that fatty liver has progressed, but they should be investigated rather than ignored.

Why Fatty Liver Can Become Serious?
| Stage | Name | Key Signs and Symptoms | Reversibility |
Stage 1 |
Simple Fatty Liver (Steatosis) |
Usually silent (no symptoms) May cause mild fatigue and slight discomfort in the upper right abdomen. |
100% Reversible through diet and lifestyle changes. |
| Stage 2 | MASH/NASH (Steatohepatitis) | Persistent fatigue and weakness, dull ache in the upper right side, and elevated liver enzymes (ALT/AST) on blood tests. | Reversible with aggressive lifestyle intervention or medical treatment. |
| Stage 3 | Fibrosis | Increased abdominal discomfort, persistent fatigue, loss of appetite, and unexplained weight loss. | Partially Reversible or manageable to prevent further scarring. |
| Stage 4 | Cirrhosis | Yellowing of skin and eyes (jaundice); swollen abdomen (ascites) and legs; easy bruising and bleeding; and confusion (hepatic encephalopathy). | Irreversible (requires medical management or liver transplant). |
Fatty liver does not always progress. For many people, lifestyle changes and management of underlying conditions can help prevent further liver damage.
“However, some people develop inflammation and liver injury. This can eventually lead to fibrosis, in which scar tissue builds up in the liver. Advanced scarring, known as cirrhosis, can permanently affect liver function and increase the risk of serious complications,” highlights Dr Dhamija.
The important point is that symptoms alone cannot tell you whether fatty liver is getting worse.
Significant liver damage can sometimes occur with few or no obvious symptoms.
How Do Doctors Check Whether Fatty Liver Is Progressing?
If you have fatty liver, doctors may use blood tests like the SGOT test, imaging, and other assessments to monitor your liver.
Liver function blood tests can provide useful information, while ultrasound or other imaging methods can help assess the liver. Depending on your individual risk factors, your doctor may also recommend tests that estimate the amount of liver fibrosis.
Regular monitoring is particularly important if you have conditions such as obesity, type 2 diabetes, or high cholesterol, which can increase the risk of fatty liver progression.
Final Word
Fatty liver can remain silent for years, so waiting for symptoms is not a reliable way to monitor the condition. Persistent fatigue, upper abdominal discomfort, jaundice, swelling, or unusual bruising can be warning signs that need medical assessment. If you have already been diagnosed with fatty liver, regular medical follow-up is important even when you feel completely well. Early detection of liver damage gives you a better opportunity to address the underlying causes and prevent further progression.
FAQ
What is the danger zone for fatty liver?
Grade 3 fatty liver is the most dangerous stage, which occurs when there is more than 66% fat accumulation in the liver. If left uncontrolled, it can lead to fibrosis, cirrhosis, and liver failure.Does a fatty liver cause a big belly?
Yes, fatty liver patients can have a big belly, as there is a strong link between visceral fat and fatty liver.
