Common symptoms include:

- Persistent crusting
- Whistling sounds when breathing
- Dryness or irritation
- Visible changes in the nose shape in severe cases
Dr Kurle adds that severe perforations often need medical or surgical repair.

Is a Perforated Septum Serious?![]()
“It depends on the size and location,” says Dr Kurle. Small perforations may cause mild symptoms. These can often be managed with:
- Saline rinses
- Humidifiers
- Nasal moisturisers
- Ointments to reduce crusting
- But larger perforations can be much more serious. They may lead to:
- Repeated infections
- Heavy bleeding
- Breathing difficulty
- Nasal deformity
- Constant crusting
“Most perforations do not heal on their own,” the doctor adds. “An ENT evaluation is always recommended.”

- Nosebleeds
- Whistling sounds
- Crusting
- Changes in nasal shape
“Once tissue is destroyed, it cannot grow back,” Dr Kurle says. “Stopping drug use completely is the only way to prevent further damage.”

How Is a Perforated Septum Fixed?![]()
Not every case needs surgery. Dr Kurle outlines the treatment options:
- Conservative care
- Saline rinses
- Nasal ointments
- Humidification
- Avoiding irritants
- These help reduce discomfort and crusting.
- Septal button
- A silicone button may be placed to temporarily cover the hole and reduce symptoms.
- Surgical repair
“For bigger perforations, surgery is often needed,” Dr Kurle says. Surgeons use tissue grafts from the nose or other areas to close the defect. But surgery only works if the underlying cause like drug use, trauma, or infection is addressed.
What Research Says About Septal Damage From Drug Use
Studies show that cocaine-induced nasal damage is far more common than people realise. A study published in The Journal of Substance Abuse Treatment noted that long-term cocaine use can lead not only to a perforated septum but also to the collapse of the nasal bridge due to tissue death. Early detection and complete cessation of drug use significantly lowers long-term complications.
