Though the storyline is fictional, Simpson’s transformation and public commentary have prompted questions about how real plastic-surgery risks play out. 

To know more about plastic surgeries gone wrong, we reached out to Dr Asma Akhlaq, Consultant Dermatologist at CK Birla Hospitals, Jaipur, to explain what “botched” really means, what can go wrong, and how to stay safe when undergoing cosmetic work.

In “All’s Fair,” Jessica’s character shares how a botched plastic surgery caused her marriage to nearly fall apart. She explains, “We’d been married almost three years. I knew he’d had affairs, so when he told me to get plastic surgery, I couldn’t say no.” The character’s flashback shows her being pressured into surgery by a musician after turning 40. 

Despite the procedure going wrong, she used the experience to fuel her dramatic revenge, symbolising the emotional toll such surgeries can have.

Dr Akhlaq defines botched surgery as one where the outcome is significantly different from the intended or safe result. “It could mean visible deformity, asymmetry, functional problems, or complications like infections, scarring, or nerve damage,” she explains. According to her, botched surgeries usually happen due to:

  • Surgeon’s lack of skill or training
  • Unrealistic patient expectations
  • Poor post-operative care
  • Unlicensed practitioners or substandard products
  • She stresses the importance of choosing qualified, board-certified surgeons and following aftercare instructions closely.

Some Procedures Are Riskier Than Others

Dr Akhlaq points out that certain cosmetic surgeries carry higher risks than others because they involve deeper structural changes. “Poor filler technique can block blood vessels, cause tissue death, or in rare cases, even blindness,”  says Dr Akhlaq.

She explains that Rhinoplasty (nose job), breast augmentation, liposuction, and facelifts are more complex and carry a greater chance of complications. Even minimally invasive treatments like Botox or fillers can go wrong if not done correctly.

What Can Be Done If A Surgery Goes Wrong?

The good news is, in most cases, there are options to fix a botched procedure. According to the exeter, the revision procedures may include:

  • Surgical correction or reconstruction
  • Scar revision or fat grafting
  • Dissolving or adjusting fillers
  • Laser-based treatments

However, she warns that timing is crucial. “We usually wait for several months for the tissue to heal before attempting any revision,” she says. “Trying to fix it too early can make things worse.”

Are Botched Surgeries Always Reversible?

Not always. While modern medicine allows many complications to be corrected, some damage can be permanent. “Issues like nerve injury, tissue necrosis, or excessive scarring may not fully recover,” explains Dr Akhlaq. “That’s why it’s critical to choose a qualified, board-certified surgeon and set realistic expectations.”

She emphasises that prevention is far safer than correction, meaning the best way to avoid a botched surgery is to do thorough research before choosing a doctor or clinic.

A study published in the Plastic and Reconstructive Surgery journal (2020) reviewed emergency visits due to cosmetic surgery complications. It found that infections, oedema, and poor technique were leading causes of adverse outcomes, emphasising the need for skilled practitioners and patient education.
