In the past decade, chronic inflammation has become a silent factor in the development of various chronic diseases. Most notably, it is a leading driver in the rise of cancer mortality, affecting almost half of the global population. While inflammation has long been associated with diabetes mellitus and heart disease, it is now considered a primary culprit in cancer development among obese individuals.
Chronic inflammation resulting from obesity is considered even more dangerous because it is often asymptomatic and leaves many confused about whether this actually triggers cancer. Obesity and Chronic Inflammation
Obesity is increasingly recognised as a state of chronic, low-grade inflammation rather than just excess body weight. According to Dr Niti Raizada, excess fat tissue, particularly visceral fat around the abdomen, functions as an active organ that releases inflammatory substances such as interleukin-6, tumour necrosis factor-alpha, and C-reactive protein.-1766752046362.jpg)
“Persistent elevation of these markers creates an internal environment that can damage DNA, encourage abnormal cell growth, and reduce immune surveillance, thereby increasing cancer risk,” she adds.
Explaining the same condition, Dr Hitesh Singhavi underlines that obesity is not just an issue of higher body mass but symbolises a condition of chronic subclinical inflammation.
“Fat tissue, especially the fat around the abdominal region, is active tissue. It produces pro-inflammatory chemicals, referred to as cytokines. It also influences hormone levels, creating a pro-inflammatory state throughout the body,” he states.
While acute inflammation is beneficial because it heals the body, chronic inflammation takes many years to result in significant tissue and cell damage.

Dr Raizada emphasises that, in addition to inflammation, obesity commonly leads to insulin resistance and elevated insulin levels. “High insulin levels promote continuous cell multiplication and suppress programmed cell death, both of which are key mechanisms involved in cancer development.”
Inflammation and Cancer Risks
Several cancers have been strongly linked to obesity-driven inflammation. These include post-menopausal breast cancer, colorectal cancer, endometrial cancer, pancreatic cancer, liver cancer, kidney cancer, and oesophageal adenocarcinoma.
“In India, the rising prevalence of sedentary lifestyles, unhealthy dietary patterns, diabetes, and fatty liver disease has contributed to a noticeable increase in obesity-related cancers, often at younger ages,” Dr Raizada says.-1766752183062.jpg)
Regarding how inflammation contributes to cancer risk, Dr Singhavi explains, “When fat cells swell, they become stressed and trigger an influx of immune cells that produce a constant inflammatory stimulus.”
These stimuli may impair DNA repair mechanisms, promote abnormal cell growth, and reduce the effectiveness of immune cells that normally destroy potential cancerous cells. Highlighting this, Dr Singhavi says, “This chronic inflammation may encourage genetic mutation, the growth of new blood vessels to fuel tumours, and the prolongation of the lifespan of abnormal cells.”
However, he also underlines that while chronic inflammation is a major risk factor, it does not guarantee one will develop cancer. The risk multiplies significantly if an individual uses intoxicants, including tobacco and alcohol, or maintains a sedentary lifestyle.

Symptoms of Chronic Inflammation
Dr Raizada shares that a major challenge is that chronic inflammation often has no clear early symptoms. However, she suggests monitoring these early signs:
- Central obesity (excess belly fat)
- Persistent fatigue
- Insulin resistance
- Metabolic abnormalities
For a formal diagnosis, blood tests such as C-reactive protein (CRP), erythrocyte sedimentation rate (ESR), HbA1c, lipid profiles, and liver function tests are typically utilised, along with imaging for fatty liver or visceral fat.

Can Reducing Inflammation Lower Cancer Risk?
According to Dr Singhavi, even a small amount of weight loss achieved through healthy eating, proper exercise, sound sleep, and stress control can decrease inflammation and increase immunity, thereby lowering cancer risk.
Clinical evidence suggests that a 5% to 10% reduction in body weight can reduce inflammatory markers, improve insulin sensitivity, normalise hormone levels, and reverse early fatty liver changes.
Regular physical activity and a nutrient-rich diet further reduce inflammation, even in the absence of significant weight loss. These lifestyle adjustments act as a powerful preventative shield against cellular damage.
