
A man struggling with suicidal ideation and depression checked himself into a mental hospital. There, he found doctors treating patients with no human connection or empathy. His condition improved with the help of another patient, but this experience led him to become the world's most well-known and humble doctor. This incident also gave birth to one of the most lasting and critical debates of the medical industry about the doctor-patient relationship.

The 1998 classic movie, Patch Adams, starring Robin Williams, talks about how laughter, joy, and compassion play a vital role in a patient’s recovery, including the treatment of critical diseases like cancer and paralysis. The movie received mixed reviews from critics at the time but later became an international success, giving weight to an important discussion around the world.
To understand how the movie’s theme is actually medically sound and whether it may cause a hindrance in treatment, we spoke to Dr Narendra BS, Lead Consultant – Endocrinology & Diabetology, Aster Whitefield Hospital, and Dr Prerna Kohli, a clinical psychologist.
Hunter Patch Adam: A Clown Doctor
Directed by Tom Shadyac, this movie is based on the real story of Dr Hunter ‘Patch’ Adams, who determines to become a doctor after experiencing the hollow treatment himself at a mental medical institute.
He is popularly known as a clown doctor for using creative methods to treat patients, whether physical or mental illness. He used to wear clown costumes and different funny attire to make patients laugh. When other health professionals did not agree with his approach, patients started showing improvements and a particular preference to get treated by him. Later, he started a home-based treatment clinic named Gesundheit (which means health in German), where he built a kind of barter system where, alongside doctors, patients also take care of each other.

The movie shows that, not satisfied with doctors' rough way of treating patients just as a body, he started challenging the traditional medical system with his own unique approach. However, he faced the State Medical Board of Virginia in the United States for his approach, as he was practising medicine without a license while still being a medical student.
In the last scene of the movie, Dr Adams gave a strong speech in front of the Medical Board and doctors against all the allegations, which changed the doctor-patient relationship approach all over the world. Dr Patch defends his actions, stating that the Doctor’s role is not only to treat the disease but to treat the patient, improving his overall health with love and dignity.
Today, even after years, his institute is serving as a non-profit organisation and provides free medical services, aiming to create a new healthcare system with a sense of community and empathy.
Doctor-Patient Relationship
Talking about Patch Adams’ approach, Dr Narendra BS stated, “One of the most important things that Patch Adams makes clear is that the compassion given to the patient can be a very powerful medicine. Human connection triggers oxytocin, lowers stress hormones, and can even help the immune system to be stronger.”
“In a situation where a patient is emotionally secure and feels understood, he or she is more likely to communicate, and this has a positive effect on the recovery process,” he further added.
Adding to the same, Dr Prerna Kohli explained, “It is a partnership, not a hierarchy. The patient brings their pain and history, and the doctor brings skill, along with their presence. Even a small moment of warmth can change how someone shows up in therapy or in a hospital bed.”

Can Empathy Interfere With Medical Treatment?
During the discussion, Dr Narendra BS mentioned that the secret lies in the existence of professional boundaries. “When a healthcare provider makes an empathetic connection but simultaneously maintains objectivity, then the results of the treatment are usually of higher quality.” The trouble only occurs when the involvement of emotions blinds one's judgment.
Dr Kohli highlights that if the doctor loses objectivity, then empathy can interfere with the treatment. “Complete detachment makes the process mechanical, and too much attachment can blur boundaries. The balance required is steady empathy,” she said.
How Does It Differ In Mental Health Patients And Physical Health Procedures?
According to Dr Kohli, “In mental health, connection is the treatment, but when it comes to other procedures, the medical procedure may take the lead, but connection still shapes recovery.”
Dr Narendra BS stated, “In the case of physical ailments, human connection may not be the cure for the disease, but it certainly brings about patient commitment, willingness, and psychological fortitude. No matter the condition, emotional validation leads to a greater sense of personal control and quality of life for the patient.”
He continued, “The movie Patch Adams tells us that the very first step of healing is through humaneness. The film illustrates the very thing that every health worker should keep in mind: that laughter, love, and emotional availability can bring strength into someone with no hope remaining."
