Liver disease is a lesser-known but possible cause of such sensations. While liver issues are commonly associated with itchy skin, they can also trigger abnormal skin sensations. Speaking to the OnlyMyHealth team, Dr Mallikarjun Sakpal, Consultant - HPB and Transplant Physician, Aster CMI Hospital, Bengaluru, explains this connection.

What Is Formication?

Formication is often referred to as tactile hallucination characterised by the feeling of insects like ants crawling on or under the skin.

It is a type of paraesthesia, also known as pins and needles, which can be caused by drug use, neurological conditions, mental health disorders, or medical issues.

Dr Sakpal says, "Individuals may find themselves grappling with this unsettling feeling due to nerve damage, which can misinterpret signals and create the illusion of insects scuttling across the skin. Additionally, mental health issues such as anxiety or substance withdrawal can exacerbate these sensations, leading to heightened awareness of bodily sensations."

Could Formication Be A Sign Of Liver Problem?

According to a 2015 study published in the journal Clinical Medicine, liver disease, particularly cholestatic liver disease like primary biliary cholangitis (PBC), can cause an intense itching sensation that some patients describe as "bugs crawling" under their skin.

Participants reported that the sensation worsened in the evening and at night, often affecting the hands, feet, and limbs.

Dr Sakpal says that the feeling of ants crawling beneath the skin can be linked to underlying liver issues. Also called formication, this peculiar sensation may arise due to the liver's crucial role in detoxifying the body and regulating various metabolic processes, he adds.

The doctor further explains, "When the liver is compromised, it can lead to an accumulation of toxins in the bloodstream, which may manifest as unusual skin sensations. Additionally, liver dysfunction can disrupt the balance of neurotransmitters, further contributing to these eerie feelings."

Other Signs Of An Underlying Liver Problem

Liver problems can cause various symptoms in the body. These include:

  • Changes in skin colour, such as jaundice
  • Changes in the appearance of rashes or unusual lesions
  • Dark urine
  • Swelling in the legs, ankles, or abdomen
  • Unexplained fatigue
  • Loss of appetite
  • Nausea and vomiting

When it comes to maintaining liver health, a healthy lifestyle plays an essential role.

Dr Sakpal recommends incorporating a balanced diet rich in fruits, vegetables, whole grains, and lean proteins. Additionally, reducing the intake of processed foods, sugars, and unhealthy fats is crucial, as these can burden the liver and hinder its ability to detoxify the body.

He further emphasises the need to stay well-hydrated by drinking plenty of water, as it aids in flushing out toxins. Moreover, irregular physical activity helps maintain a healthy weight, reducing the risk of fatty liver disease.

Most importantly, limiting alcohol consumption and avoiding exposure to harmful substances can protect the liver from damage, the doctor notes.
