Polycystic Ovary Syndrome (PCOS) is a common hormonal condition that not only affects the menstrual cycle but also leads to weight gain, excess body hair, skin problems, and, in severe cases, infertility. Additionally, women with PCOS may experience difficulty sleeping, and some may even suffer from sleep disorders like sleep apnoea, a factor that is often overlooked.

To better understand this link, the OnlyMyHealth team spoke with Dr Apurva Gupta, Consultant - Obstetrics and Gynaecology at Daffodils by Artemis, Delhi, and discussed ways to address PCOS and its related issues.

How PCOS Impacts Sleep Health?

According to a 2018 study published in the journal Nature and Science of Sleep, women with PCOS are more likely to experience sleep problems compared to those without the condition. As per the researchers, this link was not solely due to weight issues, as it exists even in normal-weight women with PCOS.

Dr Gupta attributed the issue to hormonal imbalances caused by PCOS.

She explained, “Hormonal imbalances in PCOS, such as elevated androgens and insulin resistance, disrupt the circadian rhythm and melatonin production, leading to poor sleep quality. Elevated cortisol levels due to stress also contribute to sleep disturbances.

Furthermore, symptoms such as obesity and anxiety can exacerbate sleep problems, resulting in reduced overall sleep quality and increased daytime fatigue, she adds, sharing that psychological factors can lead to a vicious cycle of poor sleep and increased stress.

Common Sleep Disorders Associated With PCOS

Several sleep disorders are commonly associated with PCOS. These include:

  • Sleep apnoea, particularly obstructive sleep apnoea (OSA), is prevalent due to weight gain and increased androgen levels that affect airway function.
  • Insomnia is characterised by difficulty falling or staying asleep.
  • Restless Legs Syndrome (RLS) is another condition linked to PCOS, often related to iron deficiency, which can cause uncomfortable sensations in the legs and disrupt sleep.

How To Manage PCOS-Related Sleep Issues

When it comes to hormonal imbalances caused by PCOS, Dr Gupta recommends focussing on regulating hormone levels through medications like metformin, which improves insulin sensitivity, and hormonal contraceptives that balance androgen levels. Lifestyle changes, such as maintaining a healthy diet, regular exercise, and stress management techniques, can also help improve sleep patterns.

As far as stress and anxiety are concerned, managing them involves strategies such as practising mindfulness meditation, yoga, and other relaxation techniques to reduce stress. “Cognitive-Behavioral Therapy (CBT) can help manage anxiety and improve sleep patterns, whereas maintaining a healthy lifestyle that includes regular physical activity and eating a balanced diet can support overall well-being,” Dr Gupta added.

Additionally, maintaining a consistent sleep schedule and creating a relaxing bedtime routine can enhance sleep hygiene.
