Dr Tyagi noted that usually patients can start climbing a few stairs four to six weeks after knee replacement surgery; however, the timing can depend on overall health, muscle strength, and the nature of the surgery. At first, one might have to use handrails or the support of a caregiver.

"Patients are typically taught to go upstairs during the rehabilitation process itself. Physiotherapists in the hospital also instruct a step-up and step-down method so that one does not put undue stress on the operated knee. But, doing stairs a little too early without proper guidance might strain the joint or even result in falls," Dr Tyagi added.

By the 8th to 12th week, most patients gain sufficient strength and balance to climb stairs independently, though they may have persistent stiffness or weakness. Complete comfort with stair climbing can take three to six months, depending upon compliance with rehabilitation.

In early recovery (first few weeks)

  • Support by using a strong handrail.

  • On descending, step down first with the operated leg.
  • Walk slowly and do not twist the knee.

As the strength becomes greater:

  • Alternate steps (one foot per step) only when the balance and stability of the knee are restored.
  • Resume physiotherapy exercises aimed at quadriceps and hamstring strengthening.
  • Use walking aids like a cane if necessary, until otherwise instructed by your doctor.

Dr Tyagi pointed out that skipping physiotherapy or going back to stairs too early are two of the most common mistakes patients make post-surgery. A proper rehab program ensures an easy transition to normal mobility without any complications.

Straight leg raises, heel slides, and mini squats can be done early on. Step-ups and resistance band exercises are eventually added with progression for hardening the knee for stair climbing.

Patients must refrain from ascending stairs if they feel:

  • Persistent swelling or pain around the knee
  • Instability or as if the knee will give way
  • Infection or inflammation within the incision region
