Hair transplant is a surgical procedure performed under the supervision of Plastic, cosmetic or reconstructive surgeons. In this surgery, the doctor usually takes existing hair from the body (donor site) and transplants it to the balding part (recipient site).

But what is the right age, circumstances, and time to plan a hair transplant? To shed light on the matter, Dr Sumit Malhotra, Director of Plastic, Cosmetic and Reconstructive Surgery, Apollo Hospital Lucknow, spoke to OnlyMyHealth.

There’s no universal 'right' age, but most hair restoration specialists recommend waiting until at least your mid-20s or later. Moreover, hair loss patterns often aren’t fully established in younger individuals,” said Dr Malhotra. 

Before consolidating your transplant plans, it is important to consider what state you are on. According to Dr Malhotra, “Hair transplant is more about the stage and stability of hair loss than age alone. If hair loss is progressing rapidly or still changing significantly, it’s usually best to wait.”

How Can Someone Tell if They’re a Good Candidate for the Procedure?

Hair transplant procedure is a typical hair surgery that requires precision and the right conditions. While harvesting and implantation sounds pretty standard in hair transplant, it is more dependent on what the patient’s hair condition is. Some major qualifications, as per Dr Malhotra, include: 

A good candidate for hair transplant typically:

  • Has stable hair loss (not actively shedding).
  • Possesses a sufficient donor area (usually the back or sides of the scalp) with healthy, dense hair.
  • Is in good overall health (no uncontrolled medical conditions).
  • Has realistic expectations, understanding the limitations of the procedure, and that results take months to mature.
  • A thorough consultation with a qualified surgeon is essential for a personalised assessment.

Is it Risky to Get a Transplant Too Early or Too Late?

As hair transplant is an elective surgery, meaning it can be scheduled as it doesn’t involve a medical emergency. “Yes, timing matters. As a personal decision, the transplant decision should be rightly timed,” said Dr Malhotra.  

Why is Timing Important?

  • Too Early: If done while hair loss is still aggressive, the transplant may not age well. Native hairs may continue to fall, creating unnatural gaps around the transplanted area.
  • Too Late: If there’s extensive baldness and a weak donor area, coverage options become limited, and results may not meet expectations.

“The right age is subjective, based on a particular patient's level of baldness and if the hairline is still receding (for men) or hair thinning (for women),” advised Dr Malhotra.

Hair transplantation is a time-sensitive procedure. Hence, the stability of hair indicates that hair loss has plateaued (change in hairline has stopped). Dr Malhotra explained why stability matters:

  • Transplanted hairs are permanent, but existing native hairs may continue to shed, negatively impacting the surgery results.
  • If hair loss progresses after surgery, patients might need additional procedures later or end up with patchy, uneven results.
  • Doctors often use tools like photographic records, scalp assessments, or medications (like minoxidil or finasteride) to monitor and stabilise hair loss before surgery.

Common Mistakes People Make With Hair Transplant?

Like any surgical procedure, mistakes exist in hair transplantation, too. According to Dr Malhotra, some frequent missteps include:

  • Ignoring Long-term Planning: Opting for a low hairline or dense transplant without considering future hair loss.
  • Skipping Medical Treatment: Not using medications to stabilise the ongoing hairloss before surgery.
  • Choosing Inexperienced or Unqualified Clinics: Leading to poor graft survival, unnatural hairlines, or scarring.
  • Overestimating Donor Supply: A limited donor area cannot cover a large bald area without compromising the natural look.
  • Expecting Instant Results: Hair growth takes 6–12 months post-procedure.

“Patients are encouraged to take a strategic, well-informed approach to ensure optimal long-term outcomes. For those considering a transplant, a detailed evaluation by a certified hair restoration surgeon is the best first step,” Dr Malhotra added.

To put it in Dr Malhotra’s words, “If hair loss is progressing rapidly or still changing significantly, it’s usually best to wait.”
