Have you ever experienced your muscle tightening up on its own? These muscle spasms known as Charley horses involve painful muscle contractions that may even cause sleep disturbance. These cramps typically occur in the calf muscles and can cause intense pain, swelling, and difficulty in movement. We spoke to Dr Rajesh Verma, Clinical Director, Orthopaedics, Marengo Asia Hospitals, Gurugram, who explained this condition, its causes, treatment, and preventive tips.

Causes of Charley Horse

Dr Verma said that Charley horses can be triggered by various factors, including:

  • Muscle Fatigue: Overuse or fatigue of muscles, often due to prolonged physical activity or repetitive movements, can lead to muscle cramps.
  • Dehydration: Inadequate fluid intake can result in electrolyte imbalances, leading to muscle cramps and spasms.
  • Electrolyte Imbalance: Low levels of potassium, calcium, magnesium, or sodium in the body can disrupt muscle function and increase the likelihood of cramping.
  • Poor Blood Circulation: Conditions, such as peripheral artery disease or sitting or standing in one position for extended periods can impair blood flow to muscles, increasing the risk of cramps.
  • Nerve Compression: Pressure on nerves from conditions like spinal stenosis or herniated discs can cause muscle spasms.
  • Medications: Certain medications, such as stimulants, antipsychotics, and diuretics, have been associated with the occurrence of Charley horses.

Symptoms of Charley Horse

The primary symptom of a Charley horse is a sudden and involuntary muscle contraction or spasm, typically accompanied by:

  • Muscle discomfort or pain in the affected area
  • Tightness or stiffness in the muscle
  • Visible twitching or bulging of the muscle
  • Difficulty moving the affected limb

Treatment and Prevention for Charley Horse

Several strategies can help alleviate the discomfort associated with muscle cramps. Here are some tips that you should follow, as listed by the expert:

Stretching

Gently stretching the affected muscle can help relieve tension and reduce the severity of the cramp. Hold the stretch for 30 seconds to a minute, avoiding bouncing movements. 

According to the Sleep Foundation, engaging in stretching or yoga before bedtime could potentially decrease both the frequency and severity of nocturnal leg cramps. Also, implementing a stretching regimen before sleep may alleviate cramps and leg discomfort within approximately six weeks.

Massage

Massaging the cramped muscle can help relax the muscle fibres and alleviate pain. To massage, gently knead the muscle in a circular motion while applying gentle pressure.

Heat or Cold Therapy

“You can even apply heat or cold to the affected area to provide relief from muscle cramps. Use a heating pad, warm towel, or warm bath for heat therapy, or apply an ice pack or cold compress for cold therapy”, said Dr Verma.

Hydration and Electrolyte Replacement

You should drink plenty of fluids, especially water, to stay hydrated and maintain electrolyte balance to avoid muscle spasm. Consuming foods rich in potassium, calcium, magnesium, and sodium can also help prevent muscle cramps.

Over-the-Counter Pain Relievers

Dr Verma added, “Over-the-counter pain relief medications, muscle relaxants, and nerve-stabilising agents can help reduce pain and inflammation associated with muscle cramps.”

Warm-Up and Cool Down

Before engaging in physical activity, warm up your muscles with dynamic stretches and gradually increase intensity. Afterwards, cool down with static stretches to promote muscle relaxation.

Avoid Overexertion

Pace yourself during physical activity and avoid sudden increases in intensity or duration, which can lead to muscle fatigue and cramping.
