Rheumatoid arthritis (RA) is a chronic autoimmune disease characterised by joint inflammation that causes pain, swelling and stiffness. It can result in joint damage, inflammation of the heart, and osteoporosis if not treated timely. Although traditional treatments often include Disease-Modifying Anti-Rheumatic Drugs (DMARDs) and Nonsteroidal Anti-Inflammatory Drugs (NSAIDs), recent studies suggest a possible link between vitamin D and RA symptom control. We spoke to Dr Narendar Dasaraju, MBBS, DNB Orthopaedics, Apollo Spectra Hospital, Chennai, who explained if vitamin D can help relieve RA.

Vitamin D and Autoimmune Diseases

Vitamin D is an essential nutrient known for its role in maintaining bone health and supporting the immune system. “Recent studies have shown a link between vitamin D deficiency and an increased risk of autoimmune diseases, including RA”, said Dr Dasaraju. According to the Centers for Disease Control and Prevention (CDC), RA is an autoimmune condition, which occurs when the immune system mistakenly targets healthy tissue, typically the joints, resulting in painful inflammation and swelling.

“Vitamin D is thought to modulate the immune response and possibly suppress the inflammatory processes that contribute to RA”, added Dr Dasaraju.

According to the Therapeutic Advances in Endocrinology and Metabolism, vitamin D deficiency is widespread among individuals with RA, and this deficiency may be associated with the severity of the disease. Vitamin D supplementation might be necessary not only to prevent osteoporosis but also to alleviate pain in RA patients.

Anti-Inflammatory Properties

One of the main mechanisms by which vitamin D can benefit people with RA is through its anti-inflammatory properties. Dr Dasaraju added, “Adequate levels of vitamin D can help regulate the production of inflammatory cytokines, substances that play a key role in the immune response. By modulating these cytokines, vitamin D can help reduce the chronic inflammation associated with RA and thereby alleviate symptoms.”

Vitamin D is important for maintaining optimal bone health by regulating calcium absorption and bone metabolism. Joint damage is a major problem in RA, and the role of vitamin D in maintaining bone density may be particularly important. 

Ensuring adequate levels of vitamin D can help maintain joint integrity and potentially slow the progression of joint damage associated with RA.

Clinical Trials

Several clinical trials have investigated the relationship between vitamin D and rheumatoid arthritis. Although the results are not consistent, some trials have shown promising results. 

A 2020 meta-analysis published in the Journal of Rheumatology found that vitamin D supplementation was associated with reduced disease activity and improved quality of life in patients with rheumatoid arthritis. However, further research is needed to determine the optimal dosage and long-term effectiveness of vitamin D supplementation in people with RA.

Dr Dasaraju highlighted, “It is important to emphasise that receiving a vitamin D dietary supplement must be approached individually. Patients with RA should consult their doctor to determine their vitamin D levels through blood tests. Based on the results, health professionals can recommend the appropriate nutritional supplement, if necessary, so that patients receive the correct dosage to address deficiencies.”

Dr Dasaraju concluded, “Current evidence suggests that maintaining adequate levels of vitamin D may help modulate the immune response and reduce inflammation associated with RA. However, patients are encouraged to work closely with their healthcare providers to assess individual needs and incorporate vitamin D supplementation into a comprehensive RA treatment plan.”
